Please download and fill out the form below. With this form, your current eye doctor can send copies of your medical records to our office for our staff to review.

In many cases, it is even better to ask your doctor to write a brief letter summarizing your medical history. This will help us greatly if his or her handwriting or records are hard to interpret.
