From Out of Town?

If you will be visiting us from out of town, please review the following information:

Plan Your Trip to Save Time and Expense 

Many patients plan a single visit to southern California where they have a consultation in our office, have surgery and the necessary follow-up exams within a short time. To do this, please contact our office by phone or through email, and our staff will be happy to assist you.

1. We will need to review your medical records. Please send this Records Release Form to your current eye doctor and request that records be sent to us directly.
   
   
2. Please send us the following items:
   — a copy of your current medical insurance card
   — filled out copies of the New Patient Information Form (2 pages).
